  • Graduate Admissions Application online or hard copy, please remember to print out the application confirmation page, you will need to include it the OT application
  • Residency Information Form, online or hard copy
  • $50.00 Graduate Admissions application fee, online or hard copy
  • Official sealed transcript from each academic institution that has been attended at the college level:

Submit to the Occupational Therapy Graduate Program (http://hsc.unm.edu/ot):

  • Occupational Therapy Graduate Program Application
  • Copy of Graduate Admissions Application Confirmation page, copies of unofficial transcripts
  • Signed Letter of Intent to apply to the Occupational Therapy Graduate Program
  • Reference Forms, from three individuals, each sealed and mailed by the individual completing the form. (Must use the references forms provided in this packet)
  • $35.00 Occupational Therapy Graduate Program fee
  • A copy of your Spring Registration, if you are still taking prerequisite courses

WICHE: If you are applying as a WICHE (Western Interstate Commission for Higher Education) exchange student, you must be certified by your state of residence. In most states, the deadline for receiving completed applications for certification is October 15 of the year preceding admission.
