Dr. Robert Sapien of the University of New Mexico invites you to participate in this questionnaire study, being conducted under the auspices of the University of New Mexico. The following will involve answering 14 questions and should take about 10 minutes to complete. Your involvement in this study is voluntary, and you may choose not to participate or to stop at any time. This questionnaire is anonymous. Your date of birth will be used for data coding only. The results of this study may be published in summary form. All information you provide will remain strictly confidential.

If preparedness varies from one of your schools to another, please provide information on the school in which you believe you have been most effective in preparing for emergencies.
