Disclosure Statement Forms

COI forms must be completed for HRRC, Pre-Award, IACUC, and RAC submissions by all investigators. Investigators are: any person (whether faculty, staff, student, consultant, or collaborator) who is responsible for the design, conduct, or reporting of UNM research. For investigators that have potential conflicts of interest, please provide additional information on a separate sheet of paper describing the potential conflict and how it may be managed. These forms are submitted again to Pre-Award for renewal/continuation applications.
