DATA SPECIFICATION WORKSHEET

Instructions: Please circle the data fields you want. Where possible please specify the following:

a) Mark specific fields versus *(all)

b) Provide date ranges

c) For diagnoses or procedures, provide specific codes (ICD-9, CPT-4, SNOMED CT) if known

d) For test codes, batteries, and testing details, see TriCore's online Test Directory

e) Provide standard terminology or other search terms in text fields

f) Specify Aggregation processes (sum, count, average, etc.) where desired

EXAMPLE:

Gout is less common in females. For preparing a grant proposal, you wish to estimate how many UNM patients, who are middle-aged Hispanic females in the greater Albuquerque area, have this diagnosis in conjunction with elevated uric acid levels. In addition, you hope to limit your activities to Albuquerque proper, unless you need to include neighboring communities to get an adequate sample size.

Your data request could be specified as follows.

In this instance, both TriCore and IDX data would used, since discharge diagnosis and ethnicity are not available in the lab-only data.
