All patients seen by the MDs and DOMs need to be referred by a primary care provider or specialist directly involved in your care, even if your insurance does not require this. This is done to ensure good communication between all providers.
Other services-such as massage, hypno-therapy, and educational classes (like MBSR and yoga) do not require a referral and are "paid-up-front" (that is, paid for with cash, check, or credit card at the time of the appointment).
For UNM Providers, use the ad hoc form in PowerChart Office after October 1, 2007. Our clinic will process the referral and call the patient.

Coverage varies by insurance carrier, type, and services. Not all services are covered by insurance or Medicare but many of our services are covered by third party insurance, so the patient is responsible for determining co-pays, co-insurance, and other financial obligations towards their insurance. Please check with your insurance carrier first. All patients with Medicare, Medicaid, third party insurance, and UNM SCI can be seen by the Integrative physicians (MD) under their plans.
A few commercial plans cover massage therapy. At CFL, massage therapy is a paid-up-front service, and then you can submit a claim to your carrier.
Center for Life requires proof of third party insurance coverage for the planned services. UNM Hospital Financial assistance is not available through the Center for Life.

10% off of self-pay services for UNM employees [UNMH, HSC, Main Campus - sorry, not for family members].
CFL is not-for-profit. Paid-up-front services are generally not covered by insurance and Medicare/Medicaid.
Patients and clients at the CFL are welcome to submit claims to their insurers for services.
Gratuities are accepted for massage therapy/esthetics/skincare/facials.

Clinic Cancellation and No Show Policy
Beginning May 12, 2009, the UNM Center for Life will be charging for appointments for which patients do not arrive to the clinic either canceling the same day or no showing for the appointment. This charge is $25.00. Should you find that you need to cancel an appointment, please call (505) 925-7464 at least 24 hours in advance or for a Monday appointments call by 4:00 PM the previous Friday. One of our staff members will be glad to cancel and reschedule your appointment.
