Minutes 1/30/07
The minutes of the Operation Committee on 24 October 2006 and the combined Forum and Operations Committee on 30 January 2007 were distributed, discussed and reviewed by the committee.
Action: The committee accepted the GME Operations Committee minutes as presented to the committee.
Old Business
-Institutional Duty Hour Violation Follow-Up was due from Neurological Surgery, and Surgery. An action plan was submitted by Neurological Surgery concerning the duty hour violation of 6/10th of an hour during last quarter. The resident was counseled and faculty will provided closer supervision in the future.
Action: The action plan submitted by Neurological Surgery was accepted as adequate. The Department of Surgery will be asked for an action plan for the reported institutional violation in December 2006.
-The University of Washington Transfer of Patient Care program is being examined by several groups at UNM HSC, such as Administration and the Cerner Work Group. The program will be offered to UNM at a low cost. The committee is planning to pursue utilization of the system at UNM. Several programs, Family Medicine, Internal Medicine, and Pediatrics, have offered to pilot the product. The product will provide more timely patient information and increase patient safety.
New Innovations – Duty Hour Strategy
Programs are expected to track duty hours, for all residents and fellows, in New Innovations effective 1 March 2007. A copy of the duty hour protocol was distributed at the last meeting. Residents and fellows are expected to enter duty hours in New Innovations by Tuesday for the prior work week. Programs are responsible to ensure compliance by all trainees. The information will be reviewed by the GMEC monthly. Programs can opt to have residents enter the information directly into New Innovations, or they can enter the residents schedule in New Innovations and have the residents verify the information. Trainings have been conducted for the institution on two dates. If programs need assistance they can contact Trey Dodson, MD of Family Medicine or Susan L. Quintana of Pediatrics. The Office of GME continues to attempt to obtain funding for a part time New Innovations Coordinator.
Callroom Allocation BBRP
The floor plans for the Barbara and Bill Richardson Pavilion which show the location of the callrooms on each floor of the facility were distributed. Emergency Medicine, ICU, Neurosurgery, and MICU call rooms will be on the second floor. The third floor is a mechanical area, but will house approximately fifteen callrooms. Labor and Delivery, and the NICU will be on the fourth floor. Pediatrics will be on the fifth floor. Pediatric ICU, and Carrie Tingley rehabilitation will be on the sixth floor. There will be approximately forty new callroom which are designated for resident physicians. Each callroom will be double occupancy and have computers, telephones, and work areas. A meeting will occur on 6 March 2007 with program directors, resident representatives, chairs, and medical directors of the above mentioned departments to allocate the space for residents and faculty for each of the services.
Some modifications have occurred on the 6th Floor Callrooms for better utilization of the space and better occupant accommodation. The rooms have been segregated for each gender. Males have two rooms and females have two rooms with a reduced number of beds in each. The two rooms at the far north east end of the area have been designated as student callrooms to be in compliance with LCME mandates.
GME Web Design Resource – HSCL
The GME Office has been able to obtain a resource within the HSC Library which will be able to provide residency programs with assistance in developing web pages for the purposes of recruitment. The referral from the program to the HSC Library should be through the Office of GME since the salary is being funded in part by money from the SOM. Standardized templates for program recruitment will be developed. Also, the computer analyst from the HSC Library will assist GME in converting the billing database to a system which interacts with Banner. The process is expected to take approximately six months. The conversion should help the Office of GME with cash flow problems.
GME Curriculum - GME Today
The UNM developed curriculum for Residents has been problematic because of access to Web CT through the HSC Library, and all the ACGME competencies are not represented with learning modules. The committee reviewed a presentation on GME Today. GME Today is a web based learning tool which has modules in all the ACGME competencies. Each competency has five to six modules which take approximately twenty minutes to complete. The product is used by a large number of teaching institutions. The cost of the product would be approximately $12,000 per year. Members were solicitated for other product suggestions. Members who wanted to review the product in greater detail were offer the opportunity to do so in GME.
Action: A more detailed demonstration of the modules will be presented at the next GME Forum, as well as discussion for a recommendation from the committee.
HO I to HO II Educational Seminar (6/29/07)
The one day educational seminar, which has been provided for residents in transition between the first and second year will be conducted at the departmental level this year. The change will provide for a more specialty specific training opportunity. Family Medicine, Internal Medicine, and Pediatrics have developed agendas for either 28 or 29 June 2007. Departments are expected to track compliance with OSHA training and the completion of the Research Design curriculum before residents sign contracts. Those contracts which are signed will not be eligible for signature by the Associate Dean until compliance can be verified. The computer lab in Med Bldg II has been reserved from 11:30-5 on 28 June 2007. Sarah Morley from the HSC Library will be available to provide assistance to any resident who needs to complete the research design modules or OSHA training.
Orientation Dates
The dates for the Intern Orientation will be 22-26 June 2007, and the date of the HO II + Orientation is 29 June 2007. The orientation schedules have been developed but have not been approved by the GMEC. As soon as they are approved they will be distributed to Program Directors and Coordinators.
-Disability Insurance for resident physicians is through Guardian. Residents can covert the policy to an individual one upon completion of training. The insurance group has asked for more direct contact with programs. The Office of GME invited Guardian representatives to the Coordinator Council and has asked they direct any departmental contact through the coordinators. There have been reports of Guardian representatives directly contacting program directors. If this process becomes problematic, access to residents may be restricted to Guardian through the Office of GME.
-Resident work space was requested by Program Directors as part of the plans to reallocate space in the old facilities when the Bill and Barbara Richardson Pavilion open. GME will forward the request to the Backfill Committee.
-Union Negotiation Committees have been formed for negotiations between the Committee of Residents and Interns and the University. The SOM will be represented by Lee Peifer, Attorney, David Sklar, MD, Associate Dean for GME, Ben Hoffman, MD, Program Director at Large, Mary Lipscomb, MD, Executive Dean, Anita Hall, and Magdalena Hall, Employee Relations Manager. The first meeting should occur in April. An UNM SOM Advisory Board for CIR union issues will be composed of Paul Roth, MD, Executive Vice President, Steve McKernan, CEO UH, and Mary Lipscomb, MD, Executive Dean. Both the CIR and UNM are entering the agreements with a positive attitude.
The next meeting will be Tuesday 24 April 2007.
