Application Instructions Applications are accepted both on-line at the University of New Mexico-Office of Graduate Studies (OGS) website (http://www.unm.edu/grad/admissions/admissions.html) and through traditional paper application. International students must follow the separate application procedures established by the University of New Mexico’s Office of International Admissions (http://www/unm.edu/preview/na_intlgrad.htm ). Application Instructions: All domestic applicants should send the following materials directly to the University of New Mexico-Office of Admissions. International applicants must apply through the Office of International Admissions. · Application: The Application/Residency Form has two parts to complete. Please ensure that all blanks on each form are filled. Missing information can create a delay in processing of the application: o Please type all information requested on the application. If not possible to type the application out, please clearly print in black ink. o Indicated your full legal name and any other names you use. If you do not have a social security number, please request one from your nearest Social Security Office (note that social security numbers will not be used as a student ID) o To complete Questions 16 and 17, enter “Clinical Laboratory Science” as the Graduate Unit (question 16) and MS-CLSC as the UNM major codes (questions 17) · Residency Form (part two of the application): All applicants are required to submit this form, whether seeking in-state residency or not. Military residents should request an Active Duty Military Tuition Residence Form from the Office of Graduate Study to be considered for in-state tuition. · Application fee: A non-refundable application fee of $50.00 must be submitted with your application. Checks or US Money Orders should be made payable to the University of New Mexico. Please do not send cash. · Transcript: One official transcript is required for every college or university you have attended whether or not you have received a degree (with the exception of the University of New Mexico; unofficial transcripts from UNM are acceptable). Do not list study abroad programs separately on the application form if they are included as part of a transcript program from an accredited US institution. Transcript must arrive in the Admissions office sealed by the registrar in order to qualify for consideration. All applicants should send these materials directly to the MLS office: · Letter of Intent: You must submit a letter of intent briefly describing your experience and objectives in undertaking graduate study in Clinical Laboratory Science (maximum of 2 pages, typed, double-spaced) · Letters of Recommendation: Three letters of recommendation are required. You should send to each person writing a recommendation on your behalf a recommendation form and envelope. The referee should be asked to return his/her sealed recommendation to you in the enclosed recommendation envelope with his/her signature diagonally across the sealed envelope flap. You should collect and send the unopened recommendations with your completed application. If the referee prefers, he/she may mail or fax (505-272-8079) the recommendation directly to the Med Lab Sciences Program Office. · GRE Scores: You must arrange to have your official GRE general Test scores sent directly to the MLS Program Office by the testing agency. (Institution code = 4845 & Department Code = XXXX). You are encouraged to include a photocopy of your test scores with the application for initial consideration; if you have taken the exam · TOEFL Scores: for international applicants must also be submitted to the Medical Lab Sciences Program. · Transcripts: One official transcript is required for every college or university you have attended whether or not you have received a degree (with the exception of the University of New Mexico; unofficial transcripts from UNM are acceptable).
