Residency Applicant Interview Scheduling Form

If you have already been offered an interview, please complete the following form to schedule your interview date (or you may send an e-mail to Hollie Medina at hmedina@salud.unm.edu):

  • Review the Interview Calendar and find an available interview slot.
  • Fill out the following form and click on the Submit button at the bottom of the page.  Your request will be forwarded to the residency coordinator.
  • A confirmation e-mail will be sent to you once your interview date is officially booked.
