Submitting an Adverse Event
When an unexpected event occurs during a research study, you should use one of the following documents to report it. Investigators must report events to the HRRC as soon as possible, but no later than 5 days from the event date or "site awareness date". For more information on Adverse Event Reporting, please see section 8.2 of our manual
