I need to Submit an Annual Review, What Do I Do?

  1. Download the Annual Review form to your desktop and complete it.
  2. Login to , and click on “Protocols” from the side menu.
  3. Click on “Active” to find your protocol.
  4. Click on the “Page” icon to access your protocol (please do not click on the “goggles” as this is a “read only” button).
  5. From the drop down list in the upper right hand side of the submissions page, select the “Continuation” title and click on “Add New”.
  6. Click on “Add Institution Forms/Supporting Documents” to upload your completed form.
  7. Click on the “Thumbs-up/Submit” icon to submit the form.
  8. You will know you are successful when the screen changes to show the routing of your submission.
