
How to Apply for Specimens
A.
If you wish to use previously banked specimens already stored in
the HTR:
a.
First, submit your proposal to the SRC (download the form from this
website).
b.
Once the SRC approves your proposal, you send the approval letter
with your application to the UNM HRRC (IRB).
c.
After HRRC approval, you next submit a tissue request form (you can
download it on this site) to the HTR.
d.
If you have tissue leftover after you have finished your project,
you must return it to the HTR for potential future use.
B.
If you wish to use paraffin embedded formalin fixed specimens that
are stored and maintained by UNM Surgical Pathology (originally removed
for patient treatment purposes):
a.
First, submit your proposal to the SRC (download the form from this
website).
b.
After SRC approval, the SRC will forward your request to the
Division Chief of Surgical Pathology to ensure availability of the
specimens while maintaining compliance with national laboratory
regulations and best patient care.
c.
Submit your proposal according to HRRC format to the HRRC and
include the approval letter from the SRC and signoff from Surgical
Pathology.
d.
If you have tissue remaining after you have finished your project,
you must return it to the HTR for potential future use.
C.
If you wish to have HTR personnel prospectively collect tissue for
a specific project:
a.
Contact the HTR for a planning/feasibility session.
b.
Submit your proposal to the SRC (download the form from this
website).
c.
Once the SRC approves your proposal, you send the approval letter
with your application to the UNM HRRC (IRB).
d.
Contact the HTR for an additional planning session (usually
required to elaborate details of the process. This meeting includes
obtaining approval from the Division Chief of Surgical Pathology to ensure
availability of the specimens while maintaining compliance with national
laboratory regulations and best patient care.
e.
If you have tissue remaining after closure of your investigation,
you must return it to the HTR for future potential use.
D.
If you wish to obtain tissues for approved clinical trials:
a.
You must obtain approval from the local HRRC if any portion of the
collected tissue may potentially be used for research.
b.
The Division Chief of Surgical Pathology or her designee must
approve the method of sample collection to ensure that sufficient
representative tissue is retained for clinical care.
