Tissue samples in category Types C will likely be from individuals followed by HSC faculty who are motivated to obtain research material for excess tissue and will have full informed consent that clearly outlines that excess tissue samples will be banked for research purposes and the linkage and recontact status of individual samples will be clearly delineated. Our proposed consent form, with embedded HIPAA authorization is included. We hope to have the approved version of this form widely distributed as an official UNMH form and to target surgical procedures that will yield large specimens that have both valuable lesions and the potential for excess tissue following the complete diagnostic evaluation. A pediatric assent form is also included for patients over 7 years of age.
