We aim to increase the quality of services by facility-based long-term care providers and reduce avoidable hospital admissions.
Using the ECHO Model, we hope to provide the opportunity for nursing home teams in rural and urban areas to share best practices in processes and procedures, receive ongoing and timely professional development, and implement collaborative problem-solving methods to help solve complex issues and questions regarding quality and value of care in New Mexico's nursing homes.
By sharing knowledge and expertise across the state in this collaborative ECHO Model, we ultimately hope to improve the health and safety of all New Mexico nursing home residents and staff.
Our program meets on the second and fourth Wednesday of the month, from 3 p.m. to 4 p.m. Mountain Time. ALL PARTICIPANTS will need to register for every session using the registration link below.

Our project is aligned with the Centers for Medicare and Medicaid Services Value-Based Purchasing requirements with a goal of helping nursing homes reduce unplanned hospital readmissions. Our goal is to have all nursing homes in New Mexico involved by 2023.

Subroto is a Program Manager at the ECHO Institute, within the NM Programs Team. His public health experience over the last 25 years spans a range of advisory, administrative, and operational roles with critical contributions to numerous domestic and global public health initiatives. In his free time, Subroto enjoys experiencing the Albuquerque hiking trails and also supporting Team Rubicon activities.
Erin is a Program Coordinator at the ECHO Institute, within the NM Programs Team. She earned her undergraduate and graduate degrees from UNM. Erin has over a decade of experience of working as a Case Manager and mostly worked with Managed Care Organizations. In her free time, Erin enjoys spending time with her husband and daughter.
