UNM Health’s Assertive Community Treatment (ACT) team located in Albuquerque provides treatment, rehabilitation and support services for people living with severe mental illness. We help our clients to live successfully, and learn behaviors to alleviate acute illness episodes.
How We Can Help
We do this by helping our clients enhance their quality of life, guide them on meeting their basic needs, and giving them tools to help them feel well enough to work and participate in other activities. We assist in easing a family’s burden of care.
Our multidisciplinary mental health team fans out through the city to help people who frequent emergency departments, jails and other long-term health care situations. The ACT team delivers psychiatric counseling and other community services to patients in their homes.
The team includes case managers, peer specialists, nurses, therapists and prescribing providers. Our professionals will meet the client in their home to collaborate on working toward recovery goals.
ACT is supported by a contract with the City of Albuquerque to serve those in our community who have struggled with traditional out-patient care and as a result frequent higher levels of emergency or inpatient care or incarceration.
