Please be aware that our office does not complete Attending Physician Statements (APS), as these are intended to be completed by the patient's attending physician. As medical examiners, our responsibility is to provide the cause and manner of death as listed on the final death certificate.
For more comprehensive information regarding postmortem findings, including a summary and opinion on how the medical examiner determined the cause and manner of death, please refer to the resources below. Here, you can find and submit a request form to obtain the autopsy/postmortem reports from our office.
Please be aware that each case is unique, the investigation process typically takes up to 90 days to complete. If you have not yet received a death certificate or autopsy/postmortem reports, it is likely because the case is still pending, and a cause and manner of death have not been determined.
