From the Subaward Recipient at the Proposal Stage

  1. A PHS 398 face page with an authorized signature from an official of the recipient institution or agency.  A formal consortium letter signed by an authorized institutional official is acceptable only when a signed PHS 398 face page is un-obtainable
    2. (as a last resort).
  2. Detailed budget with budget justification
  3. Statement of Work
  4. Biosketches of key personnel (for proposals to DHHS)
  5. Checklist page (if using PHS 398)
  6. Recipient's F&A Rate Agreement
