Tip Sheets

Our Tip Sheets are intended to give basic information about programs that may be helpful for persons with disabilities, with phone numbers and websites of where to learn more. Many Tip Sheets are in English and Spanish.
Previous, and newly developed Tip Sheets have been reviewed, updated and revised to meet Section 508 of the Rehabilitation Act of 1973 and Web Accessibility Standards. These Tip Sheets are now accessible to those with visual impairments who use screen readers like Jaws and other programs to navigate through our site.

• Child Find – Development Screenings for Children Ages 3-4 (English) - What is Child Find? The Child Find program primarily evaluates students ages 2 ½ to 5 years old who are suspected of having a disability.

• Developmental Disabilities Waiver (DD) – Allocation of Services (English) - Once a person or guardian applies for the DD Waiver, they are placed in the Central Registry. Once documentation is received to determine they meet criteria, then they are placed on the waiting list. When a spot becomes available, the person or guardian will receive a letter called the Primary Freedom of Choice (PFOC). Once this is received, a decision must be made to choose either Mi Via or Traditional DD Waiver services.
• Developmental Disabilities Waiver (DD) – Allocation of Services (Spanish) - Una vez que una persona o tutor solicita la exención por DD, se incluyen en la lista del Registro Central. Cuando un lugar esté disponible, la persona o tutor recibirá una carta llamada Libertad Primaria de Elección (PFOC). Una vez la reciba, se debe de tomar una decisión para elegir los servicios de la exención de Mi Via o por DD tradicional.

• Medically Fragile Waiver - This waiver program provides services to individuals who have been determined to have both a medically fragile condition and a developmental disability (DD). The MFW provides support
  to families by providing assistance to individuals in their homes.
• Mi Via Self-Directed Waiver (English) - The Mi Via Self-Directed Waiver is a state Medicaid Waiver program that helps people with intellectual and/or developmental disabilities self-direct their own waiver services. It allows greater flexibility to use non-traditional supports to meet needs that are directly related to their disability.
