• Intensive introduction to the UNM EMS Consortium and our 70+ partner EMS agencies. This includes travel throughout the state to introduce the fellows to our EMS colleagues in all corners of New Mexico.
• Practice and training to begin direct and indirect medical direction consultations.
• Introduction to the UNM EMS Consortium Field Response Program. This includes hands-on training in emergency vehicle operations, scene safety, fire-ground awareness, tactical medicine awareness, HazMat awareness, vehicle extrication, mountain rescue orientation, “first on scene” operations and much more.
• Familiarization ride-alongs and activities with our partner agencies so that each UNM EMS Fellow is ready to perform solo as full-fledged UNM EMS Consortium Physicians by the end of the month!

Once off and running, the rest of the year is focused on learning how to be an EMS Physician. Fellows are assigned an EMS faculty mentor who will provide 1:1 mentorship alongside the Program Director in structured and unstructured activities throughout the year.

At the beginning of the fellowship year, each fellow will be assigned to a particular fire shift to facilitate learning and the transition to a new city. Rotating cycle schedules are commonplace in the fire service and provide a perfect structure upon which to build a curriculum to train EMS physicians. Each of the program’s three fellows are assigned to one of three shifts (A, B, or C) that mirror the local fire department schedule; 48 hours on, 96 hours off cycle. The fellows remain on the same shift for the duration of the year to develop close relationships with the field crews.

We have received positive feedback from our EMS Agencies regarding the schedule and operations. Providers have commented about the educational benefits of having the same fellow on their shift for the following reasons:

• Mentorship opportunities for both the fellow and the providers.
• Familiarity with the fellow provides a comfortable atmosphere to ask questions and get feedback about patient care and skills.
• Comradery
• Streamlines patient care on scene given familiarity with fellow.
• With closer relationships and comradery, providers are more comfortable voicing concerns within the system.
• Schedule Description: During each 48-hour EMS call “shift” the fellow is available from home for medical direction phone calls and scene requests from UNM EMS Consortium agencies. During this time, the fellow will be involved in activities with faculty members that are ongoing at this time including important medical direction meetings, education, training, etc. The fellow spends a portion of each 48-hour period responding to EMS calls for their own education, to assist and interact with EMS crews, and provide on-scene quality assurance and education. After their EMS call “shift” each fellow has 48 hours off to rest. At least one of these days must be completely off, to ensure one day off every 6 days. Moonlighting is allowed as long as the fellow abides by ACGME work hour restrictions. Following the 48-hour “rest period,” fellows work a day shift in the ED at Sandoval Regional Medical Center followed by an air medical shift with UNM Lifeguard Air Emergency Services which utilizes a rotor-wing and fixed-wing aircraft.
• The Rural and Tribal EMS fellow will be heavily involved with our rural and tribal EMS partner agencies and the UNM Center for Rural and Tribal EMS but will work with urban agencies as well to round out the experiences. This balance will provide in-depth experience with all aspects of EMS in rural and tribal areas while still providing a solid foundation in urban/suburban EMS to become a well-rounded EMS physician.
• All UNM EMS Fellows will have frequent interaction with local, state, and regional EMS officials providing Fellows with a strong background in EMS policy.
• All UNM EMS Fellows are heavily involved in EMS education and training through the UNM EMS Academy and through our many partner agencies.

These are just the highlights as there are plenty of additional opportunities for the EMS Fellows to become involved with including wilderness and austere medicine, National Park Service EMS, search and rescue, tactical EMS opportunities, and more!

Didactics and ACGME Considerations

EMS Consortium meetings and program didactics are scheduled every Monday and appropriate modifications to the schedule are made to ensure fellow attendance. Didactic objectives highlight critical EMS Physician topics and prepare the fellow for the ABEM EMS Subspecialty Exam. ER shift requirements do not exceed monthly restrictions. There is always a faculty member on back-up both for supervision and to provide relief if the fellow gets called out to a scene or needs additional rest time.

Courses and Conferences

All UNM EMS Fellows are sponsored to travel to and attend the annual NAEMSP meeting in January and the National EMS Medical Director’s Course.
