Employee Occupational Health Services (EOHS) Important Forms

The Center for Occupational Environmental Health Promotion (COEHP) provides UNM employees and UNM Medical Group with occupational and environmental medicine support thorough the Employee Occupational Health Services (EOHS) Clinic. Here, employees, supervisors, and human resources professionals can find useful forms and other resources.

What to do if you are injured at work or have a condition that you believe is caused by your work:

1) Perform any first aid (eye wash, bleeding control, etc.) Call 911 for emergencies.
2) Report the injury to your supervisor and complete a Notice of Accident Form.
3) Call or bring yourself to the EOHS Clinic. We are located on North Campus at:

8:00 - 12:00 pm and 1:00 - 4:30 pm

After Hours, see a provider at the UNM Adult Urgent Care or Emergency Department.
