Closed-loop referral systems (CLRSs) seek to address both clinical and social needs by creating a two-way communication process between healthcare providers and community-based social service organizations' to ensure that patients get the support they need to maintain their health.
Santa Fe CONNECT is a CLRS that helps residents of the City of Santa Fe and Santa Fe County access needed services. Partner agencies including clinics, community organizations, and city and county programs, connect patients to resources using a shared technology platform enabling navigators to send and receive secure electronic referrals, and address residents’ social needs. This research project is helping Santa Fe CONNECT meet its goals of improving individual and community health and well-being.
Funded by the Health Care Authority (HCA), this project aims to develop an evaluation framework for the statewide CLRS, known as YES New Mexico Connect. Activities include developing a CLRS logic model, data map, and evaluation plan. The evaluation will help assess the success of expansion of YES New Mexico Connect, and identify opportunities to enhance the statewide CLRS.
