FIT Foundations of Family Service Coordination (Not-for-Credit)

Please note, this is the NOT-FOR-CREDIT version of Foundations of Family Service Coordination and does not fulfill the training requirement for new Family Service Coordinators. This course is intended only as a refresher for early intervention staff who do NOT need to receive credit for completing the Foundations of Family Service Coordination training pathway. New service coordinators should speak to their supervisor to find out when the next cohort of for-credit training will be available.

Segment 1: The Role of the FSC

• Early Intervention in New Mexico - 15 Minutes
  Overview of the mission and vision of early intervention, service delivery model, and adopted best practices in New Mexico (FGRBI, TTA, Ongoing and Authentic Assessment).
• Role of the Service Coordinator - 20 Minutes
  Introduction to the 7 steps of the family’s journey through early intervention, Knowledge and Skills for Service Coordinators, and service coordination responsibilities as they are outlined in NMAC.
• Working with Families - 30 Minutes
  Building strong partnerships with families including effective communication skills and supporting parents as advocates.

Segment 2: The First 45 Days - From Referral to IFSP

• First Contacts - 20 Minutes
  Overview of the referral process and the purpose of the  first contacts, including the initial phone call to the family and the intake process.
• Intake Part 1: Preparing for Intake - 20 Minutes
  How to balance gathering and sharing information during the “first phone call”.
• Intake Part 2: Gathering and Sharing Information at Intake - 35 Minutes
  Overview of the types of information that FSCs need to present to families and the information they need to gather during intake.
• Intake Part 3: The Family Interview - 20 Minutes
  How to conduct an effective family interview with an emphasis on collecting high-quality functional information about typical routines and activities.
• Preparing Families for the Evaluation - 12 Minutes
  How to ensure that families know what to expect during the CME process.
• Determining Eligibility - 20 Minutes
  Supporting families during the eligibility determination process.
• IFSP Part 1: Preparing the Family for the IFSP - 20 Minutes
  Supporting families to be active participants in the initial IFSP meeting.
• IFSP Part 2: FSC’s Role as a Facilitator - 30 Minutes
  Overview of general facilitation skills that service coordinators need.
• IFSP Part 3: Facilitating the IFSP Meeting - 17 Minutes
  How to facilitate the writing of the initial IFSP with an introduction to the specific components of the IFSP.

Segment 3: Ongoing Service Coordination

• Coordination of Ongoing Service Part 1: Monitoring the IFSP - 30 Minutes
  Overview of the requirements of monthly contacts, how to conduct high-quality monthly contacts, and how to support families based on information collected during monthly contacts.
• Coordination of Ongoing Services Part 2: 6-Month Review and Annual IFSPs - 20 Minutes
  The “why” of 6-month reviews and annual IFSPs and an overview of the requirements of both.
• Documentation and Billing - 20 Minutes
  A high-level overview of the requirements for service coordination documentation and billing.

Segment 4: Early Childhood Transition

• Introduction to Transition - 20 Minutes
  Overview of the early childhood transition process including an introduction to Community Based Transition Teams.
• Transition Planning - 10 Minutes
  The logistics of transition notification and an introduction to the types of transition options available to families.
• Transition Plan - 20 Minutes
  Overview of Parts 1 and 2 of the Transition Plan and their required components.
• Pre-Planning for Transition - 15 Minutes
  How to plan and prepare for the Transition Conference.
• The Transition Conference - 15 Minutes
  Purpose and requirements of the Transition Conference.
• Final Steps: Initial Comprehensive Evaluation for Part B Through Follow-Up - 30 Minutes
  Overview of the steps taken during the Part B eligibility determination and IEP processes.
