A Performance/Quality Improvement (PQI) activity is a 3-stage process of designing, implementing, and evaluating a Quality Improvement (QI) project. (Click here for ACCME's definition of Performance Quality Improvement.)
Are you planning a quality improvement (QI) or performance improvement (PI) project and seeking guidance? The forms and resources on this page are provided to guide you through the planning process so that your project leads to documented, data-based improvement, meeting your learners’ needs, and achieving the expectations for an accredited PQI activity.
Begin by submitting a simple application form about your project and the participants. Then submit an additional form at each of three milestones:
Stage A – Determining the current state and improvement need
Stage B – Designing and implementing an intervention
Stage C – Evaluating the impact of your intervention
You will begin by submitting a simple application form about your project and the participants. Three additional forms are submitted at milestone achievement points in your project:

Stage C – Evaluating the impact of your intervention
You (alone or with project colleagues) will submit each form separately as each stage is completed. There is no required timeline for completion and CME credits are provided after each stage.
1. Activity Planning: Providing basic information about your project
When completing this form (below) you provide names and contact information for project participants, the project title, disclosures of relevant financial relationships (RFR), and other general information.
2. Stage A: Learning from current practice performance assessment (5 AMA PRA Category 1 CreditsTM upon completion)
Stage A requires learning from current practice performance assessment and recognition of practice-based need. Needs are determined from an assessment of current practice, using selected performance measures, such as chart review, patient safety reports, or other appropriate mechanism. Learning occurs through the process of assembling relevant data and assessing needs. Participants are actively involved in the data collection/analysis process and report this evidence in the Stage A form (below).
3. Stage B: Learning from the application of PI to patient care (5 additional AMA PRA Category 1 CreditsTM upon completion)
In Stage B, the participants identify and implement an intervention(s) that is indicated by the Stage A analysis. Learning occurs through the process of designing the intervention. In the Stage B form (below) you will provide a detailed description of the intervention along with the rationale for the intervention design.
4. Stage C: Learning from the evaluation of the PI CME effort (10 additional AMA PRA Category 1 CreditsTM upon completion)
After completion of the intervention, you will learn from a pre/post-intervention analysis that compares results to the Stage A assessment. In the Stage C form (below) you reassess and reflect on performance in practice measured after the implementation of the intervention(s), by comparing to the original assessment and using the same performance measures. Summarize all practice, process and/or outcome changes that resulted from conducting the PI activity.
